What to bring
with you on
the day
IMPORTANT
On the day of your medical, it is ESSENTIAL that you bring the correct documents with you. If anything is missing, we will unfortunately be unable to carry out your medical. If you are unsure about anything, please call us on 0113 543 4588 and we will be happy to advise!
HGV Medical - What should I bring with me?
On the day of your HGV medical appointment, we need you to bring the following:
1) D4 Medical Form
2) Driving License
3) List of Medications: A list of any medications that you take with the doses you take.
4) If you wear glasses or lenses to drive: Please bring your Glasses/Lenses AND your lens prescription. If you do not have the prescription for your glasses/lenses, please get in touch with the optician you got them from and obtain it prior to your medical.
5) If you have any ongoing medical conditions, please bring details about them, e.g hospital letters, discharge summaries, clinic letters
Taxi Medical - What should I bring with me?
On the day of your Taxi medical appointment, we need you to bring the following:
1) Taxi Medical Form: Please bring the taxi medical form for the council you are applying for. This can usually be printed off from the council website or obtained directly from the council.
2) Your medical summary or full medical records: These are available from your GP surgery free of charge. If you are unsure about which one you need to bring, please call us on 0113 543 4588 for advice. Without a copy of your medical records, we will be unable to carry out your taxi medical.

On the day of your Ambulance Driver (C1) driver medical appointment, we need you to bring the following with you:

Crane Operator Medical - What should I bring with me?
On the day of your crane operator medical appointment, we need you to bring the following with you:
1) Identification Document (e.g driving license, passport)
2) List of Medications: A list of any medications that you take with the doses and dates you began taking them.
3) Glasses/lenses and last eye prescription
4) If you have any ongoing medical conditions, please bring details about them, e.g hospital letters
